  1. The MUM Guide to the Program Assessment Planning and Reporting  This is the guide to creating an assessment plan and to filing an annual report. Here is the link to the report form. You can’t use this to submit an annual report, but you can see the questions being asked.
  2. Assessment Plan Template.This is a downloadable template that you can use to construct your assessment plan for your academic program. Instructions are included in the MUM Guide linked above. It covers the next five years in case you wish to evaluate different outcomes in different months, but you can also evaluate all your program outcomes each year.

  4. Curriculum Mapping
    At MUM every program and learning community should have a curriculum map that shows how the student learning outcomes are related to each course in the program.
